If, for any reason, you want to change your GP practice, you will need to register as a new patient. You can find GP practices near you on the www.nhs.uk website. You will need to contact the GP practice you wish to be registered at directly.
If there are any other changes to how you contact your GP practice, for example telephone number changes, your GP practice will tell you this and updated information will be available on the practice website.
There should be no changes to how you order and receive your prescriptions from your GP practice, but if you use online services you might need to use your new registration details which will be sent to you by the practice.
